Team Leader / Care Coordinator

We’re looking for an experienced and organised Team Leader / Care Coordinator to join our growing domiciliary care service.
This is a varied role for someone who understands care, communicates well and can keep calm when plans change. You’ll work closely with the Registered Manager, carers, people using the service and their families to help make sure care is safe, reliable and person-centred.
Main responsibilities
- Supporting the full-time Care Coordinator with coordinating care calls and allocating shifts
- Helping to manage rota changes, staff sickness and urgent cover
- Covering care calls when required to help maintain continuity of care
- Completing initial assessments for new care packages
- Writing and reviewing person-centred care plans and risk assessments
- Completing spot checks and monitoring the quality of care being provided
- Supporting staff through supervisions and competency observations
- Monitoring care records and following up any concerns
- Communicating with people receiving care, their families and other professionals
- Supporting carers and offering guidance with day-to-day care matters
- Helping with staff induction, training and ongoing development
- Working closely with the Registered Manager to maintain safe, high-quality care and meet CQC requirements
- Supporting medication, safeguarding and other care-related concerns
- Taking part in the on-call rota when required
What we’re looking for
- Previous experience in domiciliary care
- Experience as a Team Leader, Senior Carer or Care Coordinator
- Good knowledge of person-centred care and safe working practices
- Strong organisation and communication skills
- Confidence using electronic care planning and rota systems
- The ability to manage changing priorities and solve problems calmly
- A full UK driving licence and access to a vehicle
- Flexibility to support the needs of the service
- A Level 3 qualification in Health and Social Care is preferred, or a willingness to work towards it
