Summary
We are seeking a Care Co-ordinator to join our team in Attleborough (NR17). Based in, this role is vital in coordinating personalized care services that empower our clients to maintain independence at home. Responsibilities
- Coordinate and manage individual care plans to meet client needs.
- Supervise and support care staff ensuring high standards of service.
- Liaise with clients, families, and healthcare professionals to deliver seamless care.
- Maintain accurate documentation and adhere to regulatory requirements.
- Use bespoke technology to schedule and monitor care delivery.
- Promote a positive team environment through effective leadership.
- Ensure compliance with health and safety standards.
- Drive continuous improvement in care services.

- Qualifications
