Witham Community Care are looking for a Care Co-Ordinator to join their small and friendly team. This is an exciting opportunity to join the management team of a small, family run Domiciliary Care Provider with a difference. We are looking for kind, caring and confident individual to help us support people to remain living in their own homes safely.
Our Care Co-Ordinator must share our vision to provide outstanding care, and preferably have experience in caring for those aged 65+ and have previously worked a similar role to a Care Co-Ordinator.
Hours of Work: 30-35 hours per week, varied
Initially 5-10 hours of these per week to be worked in Witham Community Care’s office, with the view to increase as the role expands

Key responsibilities:
- Rostering: allocating care visits to the suitable Support Worker’s in a timely manner, meaning that rotas are processed and sent to team members each week.
- Respond to any scheduling changes.
- Assist the managers to carry out client reviews in their own homes/over the phone.
- Assist the managers to carry out ‘pre-assessments’ with meeting potential new clients.
-Assist the managers to carry out staff member spot checks.
- Updating of service users care and support plans, ensuring that all information is reviewed in accordance with our policies & procedures.
- Ensuring that support plans are detailed, thorough and represent an accurate report of each service users care needs.
- To liaise with medical professionals to meet the care needs of service users.
- To liaise with NOKs, family members and friends of service users when required.
- To assist the managers with general admin duties.
- To help with induction and training of new Support Workers, introducing them to service users and ‘shadowing shifts.’
- Ensuring that the staff deliver high quality and personalised care to service users.
- Liaising with local community organisations and maintaining good relationships outside of Witham Community Care.
- Responding to emergency situations affecting service users or staffing.
- Ensuring that all activities such as personal care and medication administration meet the required standards.
- To continue to carry out care visits following the requirements of rotas/schedules.
- To cover sickness and absences and have the ability to arrange appropriate alternative schedules.
- To participate in regular on call duties and to be part of the on-call schedule. This will initially be for 7 full days, 1 week in 4.

- Excellent verbal and written communication skills and the ability to prepare comprehensive reports.
- Passion for helping people and providing personalised care.
- In-depth understanding of the organisation’s policies and processes.
- Ability to maintain a high level of confidentiality and respect for patients.
- Strong analytical skills and the ability to handle multiple tasks at a time.
- Excellent IT & computer skills, including Microsoft word, PowerPoint, excel, company systems.
- A drive for developing Witham Community Care alongside the Directors.
- Initiative to carry out duties without supervision and guide/support team members accordingly.
- An extremely reliable individual who shares our company ethos to provide outstanding care in line with Care Quality Commission and any other governing bodies.
Pay: £15.75-£16.75 per hour
