Duties:
- Provide person-centred care and support to individuals with a range of needs, supporting them to live independently in their own homes and communities.
- Assist with daily living activities, including meal preparation, personal care (such as washing, dressing, and hygiene support), and medication support in line with training, company policies, and individual care plans.
- Implement Positive Behaviour Support (PBS) strategies to promote positive behaviour and ensure the safety and wellbeing of individuals.
- Observe, monitor and report changes in an individual’s health or wellbeing to the appropriate healthcare professional.
- Collaborate with case managers, healthcare professionals, and families to support the development and delivery of individual care plans.
- Maintain accurate care records using our digital care management system, ensuring all support is clearly documented.
- Safeguard and promote the welfare of individuals by recognising and reporting any safeguarding concerns in line with company procedures.
- Promote people’s rights, independence, dignity, choice and wellbeing at all times.
- Maintain confidentiality in accordance with UK GDPR, the Data Protection Act 2018, and company confidentiality policies.
- Support individuals to attend appointments, outings, and community activities.
Qualifications:
- At least one year’s experience in a care or support role is required.
- Full induction and ongoing training will be provided.
- Understanding of care principles and commitment to person-centred support.
- Ability to communicate effectively with individuals, families, and professionals.
- Strong organisational skills and attention to detail.
- Ability to support individuals with behaviours that may challenge, using Positive Behaviour Support (PBS) approaches.
- A valid UK driving licence is preferred/required depending on service needs.

Shift Patterns:
Typical shifts may include:
- Morning shifts
- Evening shifts
- Weekend shifts
- Bank holidays (on a rota basis)
Shift patterns will vary depending on service needs and individual care packages.
Additional Requirements:
- Applicants must have the legal right to work in the UK
- All successful applicants will be subject to an Enhanced DBS check
- Satisfactory references are required prior to employment
