Job Overview
We are seeking a passionate and proactive Care Co-ordinator to join our dynamic domiciliary care team. The Care Coordinator will be responsible for organising, monitoring, and coordinating high-quality care and support services within supported living settings. The role ensures that care packages are person-centred, meet regulatory and contractual standards, and promote independence, choice, and wellbeing for people with learning disabilities, autism, mental health needs, or other support requirements.
Duties
- Develop and maintain person-centred care and support plans in line with individual needs, preferences, and risk assessments.
- Coordinate staff rotas to ensure effective cover and continuity of care.
- Liaise with service users, families, advocates, healthcare professionals, and local authorities to ensure consistent and collaborative support.
- Monitor the delivery of care and support, ensuring compliance with CQC regulations, safeguarding, and internal policies.
- Promote independence, dignity, and inclusion for all service users.
- Conduct staff spot-checks, observations, and audits to maintain high service standards.
- Assist with recruitment, induction, and training of care staff.
- Promote a positive team culture that prioritises respect, inclusion, and professional development.
- Maintain accurate records and documentation related to care performance.
Qualifications
- Proven experience in supervising or leading a care team within a domiciliary care setting
- Strong communication skills with the ability to liaise effectively with clients, families, and multidisciplinary teams
- Demonstrated ability to develop and manage comprehensive care plans tailored to individual needs
- Valid driving license preferred for transportation responsibilities or community visits
- Experience in using IT systems for record-keeping and documentation purposes
- Knowledge of safeguarding procedures and health & safety regulations in healthcare environments
- Supervising experience in a healthcare context is highly desirable

Pay: £25,420.00-£28,000.00 per year
