Location: Huddersfield
Salary: £24,920 per annum, rising to £25,926 after successful completion of 6-month probation
Contract: Permanent
Start Date: Immediate start available
Driving Licence: Essential
DBS: Enhanced Child and Adult DBS required
The Role
Venn Group is recruiting for a permanent Support Worker position with a reputable Housing Association in Huddersfield. This is an excellent opportunity for an experienced support professional to make a real difference to the lives of vulnerable adults by providing tailored support to help them maintain independence and sustain their tenancy.
Key Responsibilities
- Carry out comprehensive support assessments, risk assessments, and develop person-centred support plans.
- Provide ongoing one-to-one support to service users, regularly reviewing support plans and outcomes.
- Assist residents to access health, education, welfare, and community services, including attending appointments where required.
- Support individuals to maintain their tenancy and develop independent living skills.
- Identify safeguarding concerns and ensure appropriate action is taken in line with policies and procedures.
- Maintain accurate case notes, support records, and monitoring information.
- Build positive relationships with external agencies and partner organisations to ensure coordinated support provision.
- Encourage engagement in meaningful activities that promote wellbeing, confidence, and independence.
Requirements
- Previous experience supporting adults within a housing, supported living, homelessness, mental health, or similar support setting.
- Experience completing risk assessments and support plans.
- Strong understanding of safeguarding and supporting vulnerable adults.
- Excellent communication and interpersonal skills.
- Full UK driving licence and access to a vehicle.
- Enhanced Child and Adult DBS check (or willingness to obtain).
