We are looking for a Team Leader to join a dedicated care team supporting a young man with an acquired brain injury (ABI) from birth, developmental delay and cerebral palsy. He requires 1:1 support 24 hours a day and enjoys a varied, active lifestyle with plenty of opportunities to get out and about.
As Team Leader, you will play an important role in ensuring the delivery of high-quality, person-centred care while supporting and coordinating the wider care team.
The young man enjoys:
- Using his bath
- Swimming
- Books
- Going to the cinema
- Going out in his WAV
- Exploring the outdoors on his trike
- Going on holidays and enjoying new experiences
He attends a day centre Monday, Tuesday, Thursday and Friday, 9:30am–3:30pm, providing opportunities for staff to support with household tasks, activity preparation and planning during these periods.
Your Responsibilities
- Provide high-quality 1:1, person-centred support
- Lead and support the care team on a day-to-day basis
- Work closely with the Care Manager and wider MDT
- Help coordinate the team and ensure effective communication
- Support with staff shift cover when required
- Ensure the home is maintained to a high standard
- Support with planning and preparing meaningful activities
- Encourage and support this young man to access the community and pursue his interests
- Support with holidays and trips away
- Promote independence, choice, dignity and wellbeing
- Ensure care plans and agreed routines are followed
- Maintain high standards of care and documentation
About You
We are looking for someone who is:
- A confident and experienced care professional
- A driver with a full UK driving licence
- Over 25 years old due to vehicle insurance requirements
- Experienced in, or knowledgeable about, brain injury care (desirable)
- Knowledge of Makaton is desirable.
- Confident leading and supporting a care team
- Organised, reliable and able to communicate effectively
- Comfortable working closely with families, Care Managers and the MDT
- Flexible and willing to support with holiday arrangements and trips away
- Passionate about delivering truly person-centred care
- Comfortable supporting with personal care, including continence care. The gentleman is doubly incontinent and wears pads, so candidates should be comfortable providing personal care with dignity and respect.
Shift Pattern (DAY Shifts & NIGHT Shifts)
- Day shifts: 8:00am–8:00pm
- Sleep Shift: 8:00pm-8:00am (Incorporating an element of waking night)
