Job summary
The Willow Group are looking for a Salaried GP to join our ever expanding team. You will be part of a dynamic team responsible for providing sustainable, population-based health and social care, while ensuring our patients are seen in a timely manner by the most appropriate clinician within the Extended Primary Care Team.
The Willow Group is unique; we are partnered with Hampshire and Isle of Wight Healthcare NHS Foundation Trust, which is one of the largest Community and Mental Health Trusts in the UK. The Willow Group work over 4 Sites providing:
- GP led, working with a highly skilled team of ANPs, Nurses, Pharmacists, CPNs, First Contact Practitioners in MSK, Physician's Associate, Paramedic, Social Prescribers and supported by a motivated and efficient admin team
- Training for FY2s, GP Registrars, Nurses and Physicians Associates
- Primary Care Research
- Same Day Access Service

- A Strong MDT approach to routine care
- 15 minute appointments offering flexibility of telephone, video, on-line or face-to -face consultations

Main duties, tasks & skills required
In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care including:
Making professionally autonomous decisions in relation to presenting problems
Consults with patients in the surgery, at home visits and via telephone or on-line mechanisms
Undertakes triage calls, triage visits, checking and signing of repeat prescriptions
Screens patients for disease risk factors and early signs of illness.
In consultation with patients and in line with current Practice disease management protocols, develops care plans for health.
Provides counselling and health education as necessary.
Admits or discharges patients to and from caseload and refers them to other care providers internally and externally as appropriate
Complies with all relevant clinical governance and Practice polices and protocols.
Assists in the establishment of appropriate systems to manage common chronic medical conditions.
Records clear and contemporaneous consultation notes to agreed standards.
Provides active input to the wider Primary healthcare team in the process of health needs research/base line data collection/clinical audit.
Compiles and issues computer-generated acute and repeat prescriptions, prescribing in accordance with the Practice/Fareham & Gosport CCG prescribing formulary
Meets professional learning and CPD requirements.
