Deliver personal activity programmes for patients referred by doctors and health professionals

Conduct initial health consultations, progress reviews and assessments

Work with the Head of Health & Wellbeing to support service users towards appropriate physical activities

Help patients achieve the Chief Medical Officer's recommendation of 150 minutes weekly physical activity

Level 3 GP Referral Qualification

Knowledge of barriers preventing people from participating in physical activity and adopting healthy lifestyles

Knowledge of longer-term medical conditions and appropriate considerations for exercise participation
