Job Purpose
The Care Coordinator is responsible for supporting the safe, effective and person-centred delivery of Galiant Healthcare's domiciliary care services.
Working closely with the senior management team, care staff, people we support, their families and external professionals, the Care Coordinator will coordinate care packages and staff rotas, respond to changes in people's needs and help ensure that care is delivered safely, consistently and in accordance with agreed care plans.
The role requires excellent organisational, communication and problem-solving skills, together with the ability to manage changing priorities within a busy care environment. The Care Coordinator will play an important role in maintaining continuity of care, effective communication, accurate records and high standards of service delivery.
Key Responsibilities
Care Coordination and Service Delivery
- Coordinate the day-to-day delivery of domiciliary care services, ensuring that planned care visits are appropriately staffed and delivered.
- Prepare, maintain and update staff rotas, taking account of the needs of the people we support, staff skills and competencies, continuity of care, staff availability and travel requirements.
- Allocate suitably trained and competent care workers to care packages.
- Respond promptly to sickness, absence, lateness, missed or delayed calls and other changes that may affect service delivery.
- Arrange appropriate cover where required and escalate significant staffing or service-delivery concerns to the Registered Manager.
- Communicate changes to care arrangements clearly and promptly to care workers, people receiving care and their representatives where appropriate.
- Support the smooth commencement of new care packages and changes to existing packages.
- Help maintain continuity of care wherever reasonably practicable.
Person-Centred Care
- Promote care that respects each person's dignity, independence, choices, preferences, culture and individual needs.
- Support the implementation and review of person-centred care plans and risk assessments.
- Ensure care workers have access to accurate and up-to-date information required to provide safe and effective care.
- Identify and promptly escalate changes in a person's health, wellbeing, behaviour or circumstances.
- Liaise with the Registered Manager and relevant healthcare or social care professionals where a person's needs change or additional support may be required.
- Encourage people receiving care and their representatives to provide feedback about the quality of the service.
Communication and Partnership Working
- Maintain professional and effective communication with people receiving care, their families and representatives.
- Act as a point of contact for care workers regarding day-to-day care delivery and rota matters.
- Liaise appropriately with GPs, district nurses, social workers, local authorities, commissioners and other professionals involved in a person's care.
- Ensure important information and concerns are communicated and escalated promptly to the appropriate person.
- Build positive and professional working relationships with colleagues, external professionals and partner organisations.
Care Records and Compliance
- Maintain accurate, complete and up-to-date electronic and paper records as required.
- Monitor care documentation, including daily care notes, visit records and medication-related records where applicable.
- Identify incomplete, inaccurate or concerning records and take appropriate action or escalate concerns.
- Support the Registered Manager with audits, quality monitoring, service-user reviews and other compliance activities.
- Assist with ensuring care plans, risk assessments and service information remain current and reflect people's needs.
- Maintain confidentiality and handle personal and sensitive information in accordance with data protection requirements and Galiant Healthcare policies.
- Support the service in maintaining high standards and demonstrating compliance with relevant regulatory requirements.
Medication
- Support the safe coordination of medication assistance in accordance with individual care plans and Galiant Healthcare's medication policies and procedures.
- Monitor medication-related documentation, including MAR records where applicable, and identify any omissions, errors or concerns.
- Ensure medication concerns are reported and escalated promptly in accordance with company procedures.
- Never provide clinical advice or undertake medication duties outside the scope of the role, training or level of competence.
Safeguarding
- Remain alert to signs or indicators of abuse, neglect, exploitation or other safeguarding concerns.
- Report and escalate safeguarding concerns immediately in accordance with Galiant Healthcare's safeguarding procedures.
- Support the Registered Manager with safeguarding enquiries and information gathering where appropriate.
- Promote a culture in which the safety, dignity and wellbeing of people receiving care are prioritised at all times.
Staff Support
- Provide day-to-day operational support and guidance to care workers.
- Help ensure staff understand the requirements of individual care packages before being deployed.
- Identify concerns relating to staff performance, competence, conduct or training and escalate these to the appropriate manager.
- Support staff induction, shadowing and familiarisation with care packages where required.
- Assist with monitoring staff attendance, punctuality and compliance with operational requirements.
- Participate in team meetings, supervision, training and service-development activities as required.
Incidents, Complaints and Concerns
- Respond appropriately to service-user, family or staff concerns and escalate them where necessary.
- Ensure accidents, incidents, medication errors, missed visits and other significant events are recorded and reported in accordance with company procedures.
- Support investigations into complaints, incidents or concerns where requested.
- Contribute to identifying lessons learned and improvements following incidents, complaints or quality concerns.
On-Call and Emergency Support
- Participate in the out-of-hours or on-call arrangements where this forms part of the agreed duties of the role.
- Respond appropriately to urgent staffing and care-delivery issues during periods of on-call responsibility.
- Escalate serious incidents, safeguarding concerns and emergencies in accordance with company procedures.
Person Specification
Essential
The successful candidate will demonstrate:
- Previous experience within domiciliary care, adult social care or a similar care environment.
- Good organisational and coordination skills.
- The ability to manage staff rotas, changing priorities and competing demands.
- Excellent verbal and written communication skills.
- A professional, compassionate and person-centred approach.
- The ability to communicate effectively with people receiving care, families, care workers and external professionals.
- Good IT skills and confidence using electronic care-planning, rostering or record-management systems.
- An understanding of the importance of accurate record keeping and confidentiality.
- An understanding of safeguarding adults and the responsibility to report concerns.
- An understanding of person-centred care, dignity, independence, choice and equality.
- The ability to identify concerns, make appropriate decisions within the scope of the role and escalate matters when necessary.
- The ability to remain calm and organised when dealing with unexpected staffing or care-delivery issues.
- The ability to work independently while also contributing effectively as part of a wider team.
- A commitment to providing safe, reliable and high-quality care services.
Desirable
- Previous experience as a Care Coordinator, Senior Care Worker, Field Care Supervisor or similar.
- Level 3 Diploma in Adult Care, Health and Social Care, or an equivalent qualification.
- Experience of electronic rostering and care-management systems.
- Knowledge of CQC requirements and the regulatory framework applying to adult social care.
- Experience of reviewing care plans and risk assessments.
- Knowledge of medication procedures and MAR documentation.
- Experience of working with local authorities, commissioners, social workers or healthcare professionals.
Additional Requirements
- Appointment will be subject to satisfactory pre-employment checks, including an enhanced DBS check where required for the duties of the role.
- The postholder must have the right to work in the UK.
- Where the role requires travel to people's homes or other locations, a full UK driving licence and access to suitable transport may be required, subject to reasonable adjustments.
- The postholder may be required to work flexibly according to the needs of the service, including participation in an on-call rota where applicable.
- The postholder is expected to undertake appropriate training and maintain the skills and knowledge required for the role.
