Job title: Dementia Activities Support Worker.
1. Main purpose of the job
The main purpose of this role is to ensure the smooth running of Dementia Forward’s group work by taking responsibility for the practical arrangements but most importantly supporting and encouraging the clients who attend. The main aim of the group work is to support people with dementia and their carers to maintain wellbeing, social interaction and to assess and report any needs or concerns
2*. Position in the organisation*
- Reports to the Dementia Services Manager
- Liaises at all levels internally and with health and social care professionals externally, as well as other providers
3. Scope of the job
- Maintain responsibility to plan and carry out each group session ensuring that all the practical needs are in place.
- Ensure there is adequate manpower for each group both in terms of the numbers of volunteers and staff
- In conjunction with the team, take part in the planning and delivery of occasional fund raising activities and social events
- Provide support and mentoring to the volunteers working in the groups
4. Duties and key responsibilities
- Take a lead role in the planning and developing of the activities of the groups
- Facilitate the activities of the groups
- Assess and review the needs of those attending the groups and record and report any relevant concerns
- Make entries in client records and signpost and refer on where necessary
- Use client records to maintain an up to date knowledge of the client and their needs
- Work with the team to meet the care needs of individuals
- Actively undertake mandatory training in the following areas:- Safeguarding, Mental Capacity Act, Health & Safety at work, Policies and Procedures, Lone Working, First Aid and Food Hygiene.
- It is the duty of the Dementia Activities Support Worker to apply all of the above training knowledge and learned skills to their practice to the benefit of Dementia Forward clients/carers
- Be prepared to undertake further training should it be deemed appropriate and relevant to the Dementia Activities Support Worker role
- Provide emotional support and empathy for people with Dementia and their carers
- Work with the team to assess and monitor risk, making any necessary adjustments to practice where needed
- Maintain good communication standards and methods between clients, carers, staff and volunteers
- Provide support at events and awareness sessions where required
- Ensure clients at group events are enabled and engaged through meaningful communication and active participation
- Report any risk identified on an on-going basis and maintain sound health and safety practices for self, clients and colleagues
