We are looking for a dedicated Rough Sleeper Support Worker to become part of our Housing & Welfare team, focusing on assisting vulnerable adults who are experiencing or are at risk of homelessness. This role is crucial in addressing rough sleeping, preventing recurring homelessness, and aiding individuals with complex needs to achieve and maintain independent living. The position involves conducting outreach throughout the Walsall area, fostering positive relationships with service users, performing assessments, and offering both practical and emotional support.

Conduct outreach activities to identify and engage with rough sleepers in the area.

Follow up on referrals and deliver targeted support to vulnerable adults.

Perform comprehensive risk and needs assessments.

Develop and oversee support and risk management plans.

Assist service users with housing, welfare benefits, employment, training, and life skills.

Help individuals secure and maintain stable accommodation.

Accompany service users to appointments when necessary.

Foster positive support networks and promote community involvement.

Address crises and provide ongoing support sessions.

Make safeguarding referrals and advocate for the welfare of vulnerable adults.

Maintain accurate records, support plans, and case notes.

Collaborate closely with housing providers, support agencies, health services, and partner organizations.
Requirements:

Experience in supporting vulnerable adults, rough sleepers, homeless individuals, or those with complex needs.

Proficiency in conducting support, risk, and needs assessments.

Knowledge of homelessness, housing, welfare benefits, and support services.

Strong communication and engagement abilities.

Capability to handle challenging situations with calmness and professionalism.

Proficient IT skills and experience in maintaining case records.

Flexibility to work evenings and weekends as required.

Full UK Driving Licence and access to a vehicle for work purposes.

Experience in homelessness, housing support, outreach, supported housing, or community support services.

Experience in designing and delivering life skills or independence programs.

Experience in supervising volunteers, students, or trainees.

Knowledge of safeguarding and welfare rights.
