Job Summary

Responsibilities
- Assist patients or residents with personal care tasks such as bathing, dressing, grooming, and mobility support
- Support the development and implementation of personalized care plans tailored to individual needs
- Monitor health conditions, report changes, and document observations accurately using IT systems
- Provide companionship and emotional support to promote mental well-being
- Support with medication reminders and basic health checks under supervision
- Drive or accompany patients to appointments or outings when required, ensuring safe transportation
- Communicate effectively with team members, patients, families, and healthcare professionals to coordinate care
Skills
- Valid driving license with the ability to drive safely for home care visits or transportation needs
- Experience working in a care home or providing home care services is preferred
- Strong understanding of care plans and ability to follow detailed instructions precisely
- Proficiency in using IT systems for documentation and communication purposes
- Excellent communication skills in English, both verbal and written, to interact clearly with patients and colleagues
- Ability to work independently as well as part of a team in fast-paced environments
- Good physical fitness to assist with mobility support and personal care tasks
