Quartz Care are looking for an experienced Care Co-ordinator. This is a full time, contracted position. Job Description – Care Coordinator
Reports To: Registered Manager/ Lead Coordinator
Location: Primarily office based with some community work
Working Hours: 39 hours per week. Additionally to this you will need to participate in the on-call rota as directed by your manager (Friday 5:00pm to Monday 8:30am on designated weekends)

Job Purpose
The Care Coordinator is a key member of the care management team, responsible for ensuring high-quality, person-centred care through effective scheduling, communication, and team support. They act as a bridge between clients, carers, and leadership, helping deliver safe, responsive, and compassionate services that align with Quartz Care’s values. This role supports frontline staff, manages day-to-day operations, and contributes to service improvement initiatives.
Key Responsibilities
Care Coordination
· Develop and maintain staff rotas to ensure effective coverage, continuity of care, and responsiveness to client needs.
· Proactively manage scheduling changes due to staff absences, emergency situations, or evolving client requirements
· Communicate clearly and compassionately with care teams, service users, and families to coordinate seamless care delivery
· Work in close partnership with Case Managers to ensure care plans are accurately implemented and regularly reviewed.
· Support business growth by integrating new care packages into rotas, prioritising geographical efficiency and alignment with client preferences
· Accept and implement care packages in accordance with contractual obligations under local council framework
· Collaborate with families and stakeholders to design bespoke care packages for private clients that reflect individual values, lifestyles, and wellbeing goals
Staff Support & Supervision
- Provide regular check-ins and formal supervision for care staff
- Assist in monitoring staff wellbeing and performance
- Support onboarding of new team members, helping them settle into care duties
- Act as point of contact for day-to-day queries and coordination support
Quality Assurance & Compliance
- Assist with audits of care records, rotas, and visit logs to ensure accuracy
- Help manage document archiving and policy updates
- Maintain accurate communication records and report concerns promptly
- Participate in on-call responsibilities and manage out-of-hours care coordination
Stakeholder Engagement
- Foster positive relationships with clients, families, and multidisciplinary teams
- Promote the Quartz Care values during interactions and care planning
- Help gather feedback and identify service improvement opportunities
Professional Development
- Participate in relevant training sessions, both statutory and role-specific
- Stay informed about regulatory updates, best practice standards, and company procedures
- Uphold safeguarding principles in all aspects of coordination and team interactions

Pay: From £29,150.00 per year
