Main duties:
1. To provide person-centred, outcome-focused personal support to service users in line with Age UK Doncaster’s policies and procedures
2. To provide support and act as a role model and mentor for new and existing staff
3. To undertake community supervision with Home Care Support Workers and ensure that all staff carry out their duties in line with CQC, legal and statutory requirements
4. To maintain service user and staff records confidentially in line with Age UK Doncaster’s policy and ensure that data it is appropriately recorded, stored and transferred
5. To ensure that Home Care Support Workers have access to a supply of protective clothing and equipment
6. To undertake on-call shifts as required on a rota basis
7. To undertake quality assurance measures including service user visits and reviews
8. To liaise with service users’ important individuals and next of kin as required
9. To liaise with statutory agencies as required and at all times maintain professional relationships
10. To undertake any other reasonable duties in line with the job purpose that may from time to time be required by management.
Hours: 60 hours per fortnight. This post includes shifts between the hours of 7am -10pm on a rota basis and includes alternate weekend working. Flexibility will be required to meet the needs of the service, additonal hours above contracted hours are available delivering care in the community. You will be required to take part in the on call rota (additional on-call payments)
Pay: £14.51 per hour
