We are looking for a motivated and enthusiastic individual to join the dynamic, countywide Continuing Healthcare (CHC) team within Nottinghamshire County Council. This role is part of a diverse team carrying out collaborative work with colleagues in Health, Finance, and Service Improvement, Quality & Practice to improve outcomes for Nottinghamshire’s residents in a truly personalised way and ensure proportionate care and support is in place. Your primary focus will be attending Multi-disciplinary Teams (MDTs) to undertake Decision Support Tools (DSTs).
It is essential that you have working knowledge and experience of CHC, the National Framework, and Section 22 of the Care Act 2014. You should also have the ability to thrive in a fast-paced, dynamic environment, high-quality written recording skills, and confidence in having professionally challenging conversations. The team consists of a Team Manager, Social Worker, Community Care Officers, and Service Advisors.

You’ll need to be able to demonstrate:
- Ability to work collaboratively with people and partners, including Health, to achieve best outcomes.
- Working knowledge and experience of CHC, the National Framework, Section 22 of the Care Act 2014.
- Strong established communication skills – both written and verbal.
- Able to use a range of information technology applications, including Microsoft Teams, Outlook, Word, and Excel.
- Ability to drive and regular use of a car.
