Rawmarsh Health Centre is seeking to employ an enthusiastic, highly motivated Practice Nurse to join our current practice team. We are a provider of NHS Medical Services to a population of approximately 4,400 patients in Rotherham. We welcome the successful applicant to our friendly and committed practice team with whom they will work closely, for 25 hours a week, working over 4 days, to support the health needs of our registered population
The successful candidate must have skills in the following areas:
Chronic Disease Management (Diabetes, Asthma & COPD)
Childhood Immunisations
Cervical Smears
Family planning/sexual health
Travel Immunisations
Wound Care
Routine Injections
Job Responsibilities
To support care given by senior clinicians in the practice, by working alone and as part of the team, following practice protocols. Providing screening opportunistically combined with support and advise to patients, in order to achieve change in unhealthy behaviour, and contribute to the implementation of the National Service Frameworks and chronic disease management. To assist in clinical audit and the setting and monitoring of standards of care. Teach and direct others in the practice including unqualified staff. Participate in providing evidence-based practice and work effectively in the team.
