An exciting opportunity
Kington Medical Practice is seeking an experienced Advanced Clinical Practitioner (ACP) or Advanced Nurse Practitioner (ANP) with an Independent Prescribing qualification to become the clinical lead for our three care homes.
This is an autonomous, rewarding role for an experienced clinician with a passion for improving the care of older people living with frailty and complex health needs.
Working closely with our GP partners, care home staff and the wider multidisciplinary team, you will lead the delivery of proactive, person-centred medical care across our three care homes.
The role
You will:
- Act as the lead clinician for three care homes.
- Undertake comprehensive clinical assessments and management of residents with acute and chronic conditions.
- Independently assess, diagnose, prescribe and formulate management plans.
- Deliver proactive frailty reviews and medication optimisation.
- Provide leadership and clinical support to care home staff.
- Work collaboratively with GPs, community nursing teams, pharmacists and allied health professionals.
- Support advance care planning, end-of-life care and reducing avoidable hospital admissions.
- Contribute to quality improvement initiatives within the care home service.
About you
Essential:
- Registered Nurse with current NMC registration.
- MSc in Advanced Clinical Practice (or equivalent).
- Independent Prescriber qualification.
- Significant experience working as an ACP/ANP within primary care, community services or older people's services.
- Excellent assessment, diagnostic and clinical decision-making skills.
- Strong communication and leadership abilities.
Desirable:
- Experience leading care home or frailty services.
- Experience in medicines optimisation.
- Knowledge of the Enhanced Health in Care Homes (EHCH) model.
