Field-Based Team Leader – Full Time

As we continue to grow, we are looking for a motivated, caring and reliable Field-Based Team Leader to join our team. This is an ideal opportunity for an experienced care professional who enjoys working in the community, supporting service users and providing leadership, guidance and encouragement to care staff.
About the Role
We are seeking a Field-Based Team Leader to support our Carmarthenshire care teams.
This is a full-time role, working 5 days out of 7, with hours typically between 6:40am and 10:00pm, including breaks throughout the day.
The role is primarily field-based, with the majority of your time spent out in the community supporting service users and care staff. You will play an important role in ensuring our teams maintain high standards of care while providing practical leadership and support.
Key Responsibilities
- Providing day-to-day leadership and support to care staff across the community
- Supporting and mentoring new and existing members of the care team
- Carrying out spot checks and staff observations
- Conducting client welfare visits and monitoring the quality of care provided
- Supporting staff with queries, concerns and day-to-day challenges while out in the field
- Helping ensure care plans, policies and procedures are followed
- Promoting high standards of care, professionalism and safeguarding
- Liaising with service users, families, care staff and the management team
- Identifying and reporting any concerns relating to service users, staff or quality of care
- Supporting staff development and identifying training and coaching needs
- Assisting with maintaining compliance and accurate records
- Providing practical support to the wider management team when required
- Attending occasional office days at our Ammanford branch, normally 9am–5pm, as required
The role is designed for someone who enjoys being hands-on and visible within the care team, rather than being office-based.

Requirements
- Full UK driving licence and use of own vehicle – essential
- Minimum 1 year of care experience – preferred
- Previous team leader, supervisory or leadership experience – preferred
- NVQ Level 2 in Health & Social Care or equivalent – preferred
- Strong communication and organisational skills
- Ability to work independently while supporting and leading others
- A caring, professional and proactive approach
- Confidence when supporting and mentoring colleagues
- Commitment to delivering high-quality, person-centred care
- A flexible approach to working across the Carmarthenshire area
