Job Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. The successful candidate will play a vital role in organising and managing patient care plans, ensuring seamless communication between clients, healthcare professionals, and support services. Prior office experience is desirable to facilitate efficient administrative operations.
Duties
- Coordinate and monitor individual care plans in collaboration with healthcare providers and clients
- Schedule appointments, follow-ups, and support services to meet client needs
- Maintain accurate and up-to-date records of client interactions and care activities
- Provide occasional hands-on care when necessary.
- Act as a point of contact for clients, families, and healthcare professionals to address queries and concerns
- Organise documentation, reports, and correspondence related to client care
- Assist with administrative tasks such as data entry, filing, and managing schedules using office software
- Ensure compliance with organisational policies and confidentiality standards
- Support the onboarding of new clients by providing necessary information and guidance
What We’re Looking For
- Experience as a Care Coordinator (preferred)
- Strong organisational and communication skills
- Ability to work under pressure and manage last-minute rota changes
- Flexibility to support the service outside standard hours
- A caring, patient, and professional approach
- Full UK driving licence & access to a car (essential)
