Salary: Agenda for Change Band 7 £58,133 - £65,261 pa dependent on experience (includes HCA Supplement) plus team lead supplementary

We are looking for an experienced, enthusiastic, and highly motivated individual to take up the post of the Team Lead within our Clinical Hub Services.
This post is suited to a senior nurse with significant experience of caring for patients with life limiting illnesses who possesses advanced knowledge and skills in clinical assessment, symptom management and communication.
You will be a key member of the Clinical Hub Services, and the wider multidisciplinary team working across Royal Trinity Hospice’s community catchment area supporting patients with urgent and complex specialist palliative care needs wherever they call home.
Key Responsibilities
Work closely with other Clinical/Non-Clinical Team Leads, Heads of Services, and Director of Patient Services in providing a visible and effective leadership and management support to all staff which will achieve the goals of service and Trinity’s vision and values.
To provide clinical support to members of the Clinical Hub services
To establish and maintain relationships with external stakeholders and represent the organisation in a professional and collaborative manner.
Be part of developing and maintaining organisational systems and processes which support the delivery of high-quality integrated patient care, teamwork, and communication.
In the absence of the Community Team Lead, oversee the day to day running of the Specialist Community Nursing service
Provide clinical nursing support to the Virtual Hospice Ward
Offer support, guidance, and education to less experienced members of the rapid response team, and other professional visitors to the team to encourage individual and team learning and development in specialist palliative care.
Skills and experience required
Registered Nurse with bachelor’s degree level training in palliative/end of life care or related subject and extensive experience working within a specialist palliative care field.
Advanced assessment skills.
Advanced communication skills.
Enthusiasm and a flexible attitude to working.
Ability to work autonomously and as part of a team.
Leadership and Management Experience
The successful candidate must be able to travel with ease across our catchment area (a current UK driving licence is desirable).
