Are you an experienced and supportive manager who is passionate about developing people, improving services and reducing health inequalities?
York CVS is looking for a Social Prescribing Manager to take on an important dual role within our city-wide Social Prescribing Service.
You will lead the operational management and continued development of our Proactive Social Prescribing Service, which currently includes our Respiratory (COPD) and Trans and Non-Binary projects. You will also work across the wider Social Prescribing Service to strengthen workforce development through coaching, mentoring, professional development and service improvement.
Working closely with our Senior Project Manager and other Social Prescribing Managers, you will help ensure that our services remain safe, effective, person-centred and responsive to the changing needs of individuals and communities across York.
Your key responsibilities will include:

Leading the day-to-day delivery and development of the Proactive Social Prescribing Service.

Line managing, supporting and developing members of the Proactive Social Prescribing team.

Monitoring service performance, outcomes, quality standards and contractual KPIs.

Coordinating coaching, mentoring and workforce development activities across Social Prescribing.

Supporting recruitment, induction, retention, succession planning and staff wellbeing.

Identifying opportunities for innovation, service improvement and future growth.

Using data, feedback and learning to improve service quality and outcomes.

Building effective relationships with Primary Care Networks, NHS colleagues, VCSE organisations and other partners.

Providing operational cover for other Social Prescribing Managers when agreed and required.
We are looking for someone who can combine strong operational management with a genuine commitment to developing people and services.
You will bring:

Experience of line management, operational leadership, coaching and mentoring.

Experience of leading service development, quality improvement or organisational change.

An understanding of Social Prescribing and person-centred, strength-based approaches.

Knowledge of health inequalities, prevention and the contribution of the VCSE sector.

The ability to build effective relationships and work collaboratively across organisations and sectors.

Strong planning and organisational skills, with the ability to manage competing priorities.

The confidence to analyse information, monitor performance and produce clear reports.
-
A supportive, inclusive and solution-focused leadership style.
Experience of Social Prescribing, primary care systems such as SystmOne, monitoring and evaluation, commissioning or service sustainability would be an advantage.
You should have a degree or equivalent experience in health, social care, community development, public health, management or a related area.
York CVS delivers a city-wide Social Prescribing Service through three areas: Primary Care Network Link Workers, Frailty Social Prescribing and Proactive Social Prescribing.
Our teams work collaboratively with Primary Care, the NHS, VCSE partners and local communities to help people improve their health and wellbeing by addressing the wider social factors that affect their lives.
This role involves working at different locations across York, including GP surgeries, York Hospital, community venues and, where required, people’s homes. A driving licence and access to transport may therefore be helpful, although we welcome discussions about reasonable adjustments and alternative travel arrangements.
This is a fixed term role until 31 March 2027.
The successful applicant will be required to undertake an Enhanced Disclosure and Barring Service check.
