Advanced Clinical Practitioner (ACP) – Hythe Medical Centre | Up to £65,000 FTE
We are recruiting for an experienced AdvancedClinicalPractitioner (*ACP)* to join the clinical team in Middlesex.
This is an excellent opportunity for an autonomous ACP looking to join a supportive and forward-thinking primary care environment. The successful candidate will have the opportunity to utilise their full range of advanced clinical skills while working alongside an experienced multidisciplinary team.

The Role
As an ACP, you will undertake comprehensive clinical assessments and independently manage patients presenting with a broad range of acute, minor illness and long-term conditions.
You will have the autonomy to assess, diagnose, treat and safety-net patients within your scope of practice, with appropriate clinical support available when required.
Key Responsibilities
- Conduct autonomous face-to-face and telephone consultations
- Assess, diagnose and manage acute and minor illnesses
- Undertake comprehensive clinical assessments and examinations
- Manage patients with long-term conditions
- Request, interpret and act on appropriate investigations
- Prescribe safely within your scope of practice
- Develop appropriate treatment and management plans
- Provide effective safety-netting and follow-up
- Identify and appropriately escalate deteriorating or acutely unwell patients
- Make appropriate referrals to secondary care and other services
- Support preventative healthcare and health promotion
- Maintain accurate and comprehensive clinical records
- Work collaboratively with GPs and the wider multidisciplinary team
- Contribute to clinical audits, quality improvement and service development
About You
We are looking for an experienced ACP who is confident working autonomously within primary care and able to manage a varied patient caseload.
Essential:
- Registered Nurse or Allied Health Professional with relevant professional registration
- Advanced Clinical Practice qualification or equivalent
- Independent Prescribing qualification where applicable to professional background
- Previous experience working within primary care
- Strong clinical assessment and diagnostic skills
- Confident managing acute presentations
- Excellent consultation and communication skills
- Ability to make autonomous clinical decisions
