We are looking for a motivated and dedicated individual to join our close knit, supportive team within the Mental Wellness Team, part of the Vulnerable People’s Service, as a full-time Assessing and Reviewing Officer.
This is an exciting opportunity to work within a social work mental health setting, supporting individuals with severe and enduring mental health conditions that impact their daily functioning and quality of life. The role involves working closely with individuals, their families and carers, including those who have recently been discharged from hospital following detention under the Mental Health Act. You will work in partnership with health colleagues in Community Mental Health Teams, using a collaborative and person-centred approach with a strong focus on recovery, reablement and outcome-based planning.
The role will also involve carrying out Section 117 reviews to meet the Council’s duty under the Mental Health Act, ensuring that people who have been detained in hospital receive the right support and aftercare when they return to the community
You will be supported through regular formal supervision, day-to-day guidance and a range of ongoing development opportunities, alongside a strong team ethos that values support, learning and professional growth.
We are looking for someone who can demonstrate knowledge and experience within the social care and/or healthcare sector, ideally within a mental health setting, and who holds a relevant qualification such as NVQ Level 3 in Health and Social Care or equivalent. Strong communication, organisational and partnership working skills are essential, along with a compassionate and person-centred approach.
The role requires the ability to travel regularly across the County, including to remote locations, sometimes at short notice. Due to the nature of the work, a satisfactory Disclosure and Barring Service (DBS) check will be required.
