We are seeking an enthusiastic and motivated Registered Nurse (Band 7) to join our forward‑thinking Pre‑assessment and Population Health Management Team at West Middlesex Hospital.
This is a unique and exciting opportunity to establish and develop the Trust’s first Population Health Management Clinic , while also playing a key leadership role within the Pre‑assessment (POA) service . The post holder will work collaboratively with hospital and community teams to deliver a high‑quality, patient‑centred service aimed at improving both individual surgical readiness and wider population health outcomes .
The role combines clinical expertise, leadership, and service development , with responsibility for assessing patients prior to surgery, identifying and managing risks, and ensuring patients are optimised for safe procedures. This includes reviewing medical history, performing clinical assessments, arranging appropriate investigations, and providing education to support informed decision‑making.
In parallel, the post holder will lead on prevention, early intervention, and lifestyle modification , supporting patients to reduce risk factors such as smoking, poor diet, and inactivity, thereby improving long‑term health outcomes and reducing health inequalities .
- Undertake comprehensive pre ‑ operative assessments in line with agreed protocols
- Identify and manage clinical risks, ensuring patients are optimised for surgery
- Request, interpret, and act on investigations (e.g. blood tests, ECGs)
- Provide patient education on procedures, recovery, and lifestyle improvements
- Deliver and develop population health interventions, including behaviour change support
- Work collaboratively with community public health teams to support continuity of care
Contribute to the development of the nurse ‑ led population health clinic

Roles and responsibilities
Pre‑assessment Clinical Responsibilities
Assess the health status of adult elective surgical patients, obtaining a comprehensive medical, psychological, and social history, involving carers/relatives where appropriate
Perform clinical assessments to determine fitness for anaesthesia and surgery, making autonomous decisions regarding suitability to proceed
Undertake clinical interventions such as phlebotomy and ECG recording in accordance with Trust policies
Request, perform, and interpret investigations in line with agreed protocols and multidisciplinary guidance
Demonstrate advanced analytical and clinical judgement to develop appropriate care plans
Identify and manage perioperative risks, including co-morbidities and medication optimisation
Provide clear, accurate information to patients regarding procedures, risks, benefits, and recovery , supporting informed consent
Discuss discharge planning and set expectations, particularly for day-case surgery pathways
Contribute to the development and implementation of pre-assessment protocols, pathways, and guidelines
Review and evaluate results of investigation/care promptly to ensure patient’s condition and safety is optimised preoperatively to avoid inappropriate admissions and delays in surgery being performed.
- Directly refer patients to health care professionals to ensure appropriate assessment and evaluation prior to admission in collaboration with the multidisciplinary team.
- Maintain up to date standard documentation in accordance with the NMC Standards for Records and Record Keeping.
- Act in accordance to the NMC Code of Professional conduct and guide others to do the same.

- Contribute to improving patient care via inter-disciplinary decision making.
Population Health and Preventative Care Responsibilities
Receive referrals from pre-assessment services for lifestyle and health behaviour interventions
Communicate effectively with patients regarding health conditions, treatment plans, medications, and disease processes
Provide tailored education and support to enable patients to self-manage long-term conditions such as diabetes, hypertension, and asthma
Deliver interventions focused on smoking cessation, diet, physical activity, and alcohol reduction
Conduct health risk assessments and screenings , identifying opportunities for early intervention
Promote preventive health measures, including screening uptake and regular check-ups
Encourage behaviour change to reduce surgical risks and improve long-term outcomes
Attend population health meetings and present activity reports as required within the hospital as well as in the community.
Developing and maintaining relationships with patients, including conducting follow-up visits, communicating with patients’ doctors, and encouraging patients to adhere to treatment plans as required.
Helping patients manage their health conditions by providing advice on diet and exercise, arranging follow-up appointments with specialists as needed, and recommending community resources such as social workers or physical therapists.
Coordinating with other members of the healthcare team, including physicians, nurses, pharmacists, nutritionists, and social workers
To educate patients about preventive health measures, such as regular check-ups and screenings, and to help them understand how their behaviours affect their health.
Prepare monthly report about all activities of the population health clinic and out-comes of the referred patients.
Conduct health risk assessments and screenings, providing education and referrals as needed.
Develop and implement population health initiatives aimed at improving the health of defined patient populations.
Monitor and evaluate the effectiveness of population health interventions.
Collect and analyse data to identify trends and areas for improvement in population health.
Serve as a resource to other nurses and healthcare professionals on population health topics
Advocate for policies and practices that improve population health
Collaborate with community partners to develop and implement programs that address population health management.
Disseminate information on best practices in population health
Participate in research studies related to population health
Maintain up-to-date knowledge of population health trends
