19 Hills Health and Wellbeing Centre opened in January 2025,
bringing a range of services for patients together in one place, including General Medical Services, Community Dentistry,
Community Services, Social Care and Third Sector.
The HWBC provides an opportunity to address the whole needs of the
community in terms of keeping people well, supporting them to manage their health conditions, wellbeing, reduce loneliness and
promote independent living, all in a welcoming environment.
An exciting opportunity has arisen for a fixed term Community Information Officer to join the Newport Locality Team to address local health inequalities and support the development of community wellbeing activities for the local population.
The Community Information Officer will support patients and members of the community to access services and provide information and support from 19 Hills HWBC. and will encourage and facilitate activities within the HWBC, responding to community need and engage with partners and stakeholders; creating a bustling and busy environment that allows all users needs to be catered for.
This post is a fixed term/secondment for 8 months until 31 March 2027 due to funding. If you are interested in applying for the secondment position, you must obtain permission from your current line manager prior to applying for this post.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
The post holder will:
- Support patients and members of the community to access services and provide information and support from 19 Hills HWBC.
- Encourage and facilitate activity within the HWBC, responding to the community need and engaging with a wide range of stakeholders and partners.
- Encourage innovation and create a bustling and busy environment which is safe and inclusive and allows all users’ needs to be catered for.
- Contribute to develop local knowledge and skills, using assets-based approaches and support the Place Based Well-being Collaborative to fully involve the community.

Engage communities in health and well-being activity and work in partnership with others that provide these opportunities
Support and empower patients, visitors and services by providing accurate information, advice, and guidance on local
opportunities, services, and resources, with the aim of improving wellbeing, promoting independence, and reducing loneliness
and social exclusion.
Encourage and facilitate activity within the HWBC, responding to community and service delivery need and engaging
with a wide range of stakeholders and partners.
Promote and support activities within the HWBC and set up and prepare areas for community use
Support with patient and visitor wayfinding within 19 Hills HWBC.
Help to identify gaps in service provision within the HWBC, by working alongside services and patients and identify local wellbeing activities to complement the existing services within the building.
Responsible for maintaining and updating the "Your Newport Your Wellbeing" website
