Community Health Care Assistant to support South Wirral Community Nursing Team.
A Community Nursing Auxiliary/ Health Care Assistant (HCA) in the National Health Service (NHS) plays a key role in supporting patients in their own homes or community settings. Their duties focus on practical care and assisting qualified healthcare professionals.
Here are the main duties :
Monitoring Clinical Observation signs (temperature, pulse, respiration, blood pressure)
Supporting medication routines (depending on training and supervision)
Dressing wounds or assisting nurses with procedures
Helping patients maintain a clean and safe home environment
Supporting with light household tasks (e.g., meal prep, tidying)
Encouraging independence in daily activities
Providing companionship and reassurance
Supporting patients with mental wellbeing and reducing isolation
Communicating with patients’ families or carers
Recording patient information and reporting changes in condition
Communicating with nurses, GPs, and other healthcare professionals
Following care plans accurately
Travelling between patients’ homes
Working in diverse settings like clinics, care homes, or supported housing
Following infection control procedures
Recognising and reporting safeguarding concerns (e.g., abuse or neglect
