The position will be for the successful applicant to work within the Carrick iCMHT based in Truro and covering the Carrick district including surrounding areas, St Agnes and Falmouth. The successful applicant will be expected to work in the Community with individuals with Severe and Enduring Mental Health Conditions.
You will be part of a multi-disciplinary team, contributing to the effective provision of Mental Health Services to an identified caseload of service users.
You will provide a range of services including practical and emotional support to individual service users as indicated by the individual care plans provided by their lead health care professionals.
This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation, therefore driving is essential to for this role.
This job is currently only open to people who already work for the NHS in Cornwall or the Isles of Scilly.
