Overview
To provide practical, person-centred support that improves the health, wellbeing, confidence, independence and social inclusion of individuals who use INCIC services. The postholder will help plan and deliver accessible activities, provide one-to-one and group support, respond appropriately to identified needs, and work with colleagues and partner organisations to help people access relevant services and opportunities.
The role supports people with a range of physical, emotional, mental and social wellbeing needs. It does not involve providing clinical diagnosis or treatment. The postholder must work within their competence, follow agreed support plans and procedures, and seek guidance from the Health & Wellbeing Manager / Team Leader whenever needs or risks fall outside the scope of the role.
Duties
- Deliver tailored health and wellbeing support to clients, focusing on their individual needs and goals
- Assist with daily activities that promote physical health, mental resilience, and nutritional well-being
- Support clients in understanding their health conditions and navigating healthcare services effectively
- Conduct public speaking sessions or group workshops to raise awareness about health issues and healthy habits
- Collaborate with healthcare professionals to develop personalized care plans and monitor progress
- Maintain accurate records of client interactions, progress notes, and relevant health information
Qualifications
- Experience or background in nursing, public health, or related healthcare fields is highly desirable
- Excellent communication skills with the ability to engage diverse audiences through public speaking or teaching
- Compassionate attitude with a genuine desire to support others’ wellbeing
- Ability to work independently and as part of a multidisciplinary team
- Prior experience in community outreach or public health initiatives is a plus
- Relevant certifications or training in healthcare or wellbeing support are advantageous
