Working with a community nursing team, you’ll provide expert care and coordination for people with complex health needs, helping them stay well and receive the right support at the right time. Based within a multidisciplinary service, you’ll play a key role in delivering high-quality care for patients with long-term conditions while helping to prevent unnecessary hospital admissions.
You’ll assess, plan, deliver and review care, working closely with patients, carers and partner organisations. You’ll also support colleagues, contribute to service improvement and help maintain high standards of clinical practice across the team.
What you’ll do:
Manage a caseload of patients with complex nursing and long-term condition needs
Assess, plan and coordinate care, ensuring patients receive safe and effective support
Work with patients and carers to promote independence and self-management
Collaborate with health and social care professionals to deliver joined-up care
Support and supervise colleagues while contributing to service improvement and best practice
What we’re looking for:
Registered Nurse qualification and current professional registration
Experience delivering care for people with long-term conditions and complex nursing needs
Strong assessment, communication and care coordination skills
Ability to work independently and as part of a multidisciplinary team
Commitment to continuous professional development and high-quality patient care
