This is an excellent opportunity for an ambitious and innovative Broker to join our Brokerage Team.

Working creatively with individuals, local communities and our partners, we empower people to achieve the best possible outcomes while ensuring resources are used effectively and sustainably.
As a Broker, you will work alongside Adult Social Care practitioners, managers, partners and providers to support individuals in meeting their assessed needs. You will be responsible for sourcing appropriate care and support solutions, negotiating with providers and identifying cost-effective services that deliver positive outcomes and represent value for money.
You will develop and maintain strong working relationships with providers, social care teams and commissioning colleagues, using your knowledge of local services and the wider care market to secure timely and effective support. You will help maximise the use of commissioned services, identify creative solutions where capacity is limited, and support practitioners to provide choice and control to individuals accessing services.
You will need to be a self-motivated individual with excellent organisational skills, capable of managing competing priorities in a fast-paced environment. You will be confident working both independently and as part of a team, maintaining accurate records and ensuring that services are arranged in accordance with Council policies, safeguarding requirements and regulatory standards.
The role also involves working with providers and social care teams to support timely hospital discharges, prevent unnecessary admissions, identify gaps and pressures within the local care market, and contribute to the delivery of high-quality services that support people to remain independent within their communities.
We will provide you with high-quality supervision, ongoing support and training to enable you to develop your skills and progress within Adult Social Care Brokerage.
You must be fluent in the English language (as a requirement of Part 7 of the Immigration Act “for the effective performance of a customer-facing role”)
The team operates a hybrid working model with all practitioners expected to be in the office for a minimum of two days per week.
You will be subject to an enhanced Disclosure and Barring Service Check.
