What we’re looking for
This is an exciting opportunity to join a fast-paced and growing organisation.
We’re looking for Navigator Practitioner for our VIA Greenwich Service who is a dynamic and driven individual who is committed to providing assertive outreach to service users who can help engage the homeless population into wellbeing services and drug and alcohol treatment. Someone who is able to deliver satellite provisions within local services e.g., GPs, probation, hostels etc. You will be able to work both independently and as part of a supportive team to carry out assessments, risk management and develop person-centred Recovery Care Plans predominantly with homeless service users, to deliver harm reduction advice, well-being, education, and support. Someone who is able to conduct street outreach to engage/re-engage homeless individuals misusing substances.
