Support Worker - Assisted Discharge & Support at Home service

Salary: £24,479 per annum, pro rata (plus ILW, if residing & working in London)
Hours: 35 per week
Contract: Fixed term contract (until 31st March 2027 with possible extension subject to funding)

As a Support Worker, you'll play a vital role in helping people regain their confidence and independence after leaving hospital. Every day is different. You might accompany someone home following discharge, help them access essential shopping, carry out welfare visits, or simply be a reassuring presence during a challenging time. Your support can make a lasting difference to someone's recovery and wellbeing.
Based primarily at Chelsea and Westminster Hospital, you'll support people across the local community, helping them recover safely at home and reducing the likelihood of hospital readmission. Depending on service needs, you may also provide occasional cover at other hospital sites across London.

What will a day in the life of a Support Worker involve?
- Supporting adults following discharge from hospital to help them regain confidence and independence at home.
- Accompanying service users home from hospital and helping them settle safely back into their home environment.
- Providing practical and emotional support such as shopping assistance, welfare visits, companionship and connecting people with local services and activities.
- Assessing individual needs and following personalised support plans to achieve positive outcomes.
- Carrying out welfare checks and reporting any changes in a service user's wellbeing.
- Working closely with clinicians, health professionals and community partners to ensure people receive the support they need.
- Maintain accurate records, update case management systems, and complete a range of administrative tasks to support effective service delivery.
- Travelling throughout the local area to provide support wherever it is needed.
To be a successful Support Worker, what will you need?
- A caring, positive and empathetic approach.
- Excellent communication and people skills.
- The ability to work independently and as part of a supportive team.
- Good organisational skills and the ability to adapt to changing priorities.
- Basic IT skills and a commitment to maintaining accurate records.
- A willingness to travel regularly across the local community as part of your role.
- A good level of physical fitness and mobility. This is an active role and may involve accompanying service users on journeys, carrying shopping or essential items, and spending significant time travelling between locations.
- A willingness to learn, develop and undertake training as required.
