We are looking for Rehabilitation Support Worker (RSW) with a positive attitude and commitment to providing excellent holistic care. The post holder will provide support to patients in order to enable activities such as washing and dressing, preparing their meals, carrying out exercise programmes, walking, getting out into the community, access public transport etc. Care plans are prescribed by qualified therapists and you will be supported to deliver these programmes by the professional staff.

The Rehabilitation Support Worker (RSW) functions as part of the Multi-Disciplinary Team to provide a seamless service to a group of patients under the guidance and supervision of an Allied Health Professional.
The RSW will be expected to become proficient and competent in their area of practice by undertaking competency assessments specific to the area of clinical practice.
The RSW will promote optimum independence and individualised patient-centred care to a delegated caseload.
The RSW will also support the team to deliver against key performance indicators and maintain quality of the service
