We have an exciting opportunity for a qualified practitioner (Nurse/ OT/ Social Worker/ Mental Health Wellbeing Practitioner) to work within the Neighbourhood Mental Health Team in Heywood, Middleton and Rochdale.
We welcome applications from practitioners who hold additional psychological qualifications or are a qualified Mental Health and Wellbeing Practitioner (MHWP), with the ability to deliver psychologically informed interventions. This is a 6‑month fixed-term contract within the team, working across GP surgeries to support individuals aged 18 and over with severe and enduring mental health conditions, as well as those presenting with trauma and complex needs.
The post holder will play a key role in providing advice and triage, carrying out clinical assessments where appropriate, and offering short-term interventions for individuals experiencing complex mental health difficulties. Where required, you will facilitate referrals and transitions into the most suitable services within the transformed Community Mental Health Service.
In this role, you will be part of an experienced, knowledgeable, hardworking, and highly supportive multidisciplinary team.
We currently have a vacancy for a full time Band 5 Community Mental Health Practitioner within our HMR Neighborhood Mental Health Team.

This is a full time post working core hours of Monday to Friday, condensed hours over 4 days will be considered.
PARIS training would be an advantage however training will be provided if required.
