We believe everyone deserves the opportunity to move forward and reach their full potential.SummaryWe are looking for a passionate and motivated Resettlement Support Worker to join our growing team. This rewarding role involves supporting individuals leaving prison, people on probation, and vulnerable adults living within supported accommodation and the wider community.You will play a key role in helping service users overcome barriers relating to housing, employment, education, wellbeing, independent living, and social inclusion while supporting them to achieve positive and sustainable outcomes.Responsibilities
- Manage a caseload of service users and provide high-quality, person-centred support.
- Complete assessments, support plans, risk assessments, and regular reviews.
- Deliver one-to-one keywork sessions focused on achieving agreed outcomes.
- Support service users to access accommodation, benefits, healthcare, employment, training, and community services.
- Promote independent living skills, wellbeing, and positive lifestyle changes.
- Work closely with Probation Services, Local Authorities, housing providers, healthcare professionals, and partner agencies.
- Conduct welfare checks and contribute to safeguarding and risk management processes.
- Maintain accurate records, case notes, reports, and outcome data.
- Attend team meetings, supervision sessions, and multi-agency meetings.
- Encourage engagement, rehabilitation, and successful community reintegration.
Qualifications & ExperienceEssential
- Experience working within homelessness, supported housing, probation, criminal justice, mental health, substance misuse, or social care services.
- Experience managing caseloads and supporting individuals with complex needs.
- Strong communication, organisational, and interpersonal skills.
- Ability to build positive professional relationships and maintain appropriate boundaries.
- Good IT skills and experience maintaining accurate records.
- Understanding of safeguarding, risk management, and confidentiality.
Desirable
- Knowledge of the criminal justice system and probation services.
- Experience supporting individuals leaving prison or subject to community supervision.
- Knowledge of housing, welfare benefits, and tenancy sustainment.
- Understanding of trauma-informed and strengths-based approaches.
- Relevant qualification in Health & Social Care, Criminal Justice, Psychology, Housing, Social Work, or a related field.
