Community Care and Support Worker
We are looking for compassionate and dedicated Community Care and Support Workers to support people with Learning Disabilities, Autism, Mental Health needs, Sensory Impairments and Physical Disabilities to thrive within their own homes and communities.
The role is centred around promoting independence, choice and control, empowering people to live fulfilling lives whilst receiving person-centred support tailored to their individual needs and aspirations.
Responsibilities include:
* Providing person-centred support within people’s homes and local communities.
* Promoting independence and developing daily living skills, including cooking, budgeting, shopping and maintaining a safe home environment.
* Supporting people to access education, employment, volunteering and meaningful social activities.
* Providing emotional and practical support to promote positive mental health and wellbeing.
* Supporting individuals with communication needs, sensory differences and physical disabilities in a respectful and inclusive manner.
* Encouraging people to make informed choices and achieve their personal goals and outcomes.
* Working collaboratively with families, health and social care professionals and other agencies to ensure coordinated support.
* Maintaining accurate records and contributing to support plans and risk assessments.
* Upholding safeguarding responsibilities and promoting dignity, respect and equality at all times.

Pay: £19,827.60-£20,000.00 per year
