Working with a multidisciplinary team, you’ll deliver timely support so patients receive safe, effective care when they need it most. You’ll help people experiencing an acute health crisis, supporting them in community settings and their own homes. Your role is key to keeping care joined up, helping patients stay comfortable, informed, and safe
What you’ll do:
Support patients with clinical tasks such as observations, blood sampling, ECGs and basic care needs
Carry out pre-assessment calls and gather key information before appointments
Monitor patients’ condition and promptly escalate concerns to the clinical team
Work closely with colleagues across health and care to deliver coordinated support
Keep accurate records using care plans and electronic systems
What we’re looking for:
Experience in healthcare, social care, or a similar support role
A relevant Level 3 qualification or willingness to build skills through training
Good communication skills and the ability to work as part of a team
Confidence using IT systems and maintaining clear records
A flexible approach to working patterns, including weekends
