About the role
A new and exciting opportunity has arisen to lead on the embedding of an enablement approach within mental health social work.
In this role, you will develop, implement and review a new approach to building the independence of people diagnosed with mental health issues who require social care services from Cambridgeshire County Council. This will involve reviewing current arrangements, designing and implementing service transformation and identifying areas for improvement.
You will work across Adults and Older Adults teams, including transitions.
This post will require you to review individuals referred and then carry out an initial baseline assessment to determine their needs. You will then work with commissioners, contracts, providers, Early Help, TEC and health teams to develop an enabling approach for those individuals and will then carry out reviews and benchmarking to see if there is any change in the individual’s personal outcomes and level of care and support. You will have the opportunity to shape the role and create standard operating procedures to embed this practice across our teams.
Working arrangements
You will be supported to work flexibly with the opportunity to work some from home and some office based. You will have management and peer support across our mental health social care teams. This role will require you to travel across the County when you need to meet the individuals that you are working with, who will be from a diverse range of backgrounds with a variety of diagnosed conditions.
