Community Care Partner
Location: Hybrid – Office based in Aberdeen with regular travel throughout Scotland

The Role
As a Community Care Partner, you will play a key role in ensuring our customers receive outstanding care while supporting and developing our Care Companions. This is a varied hybrid position combining office-based responsibilities with regular travel across Scotland to visit customers and Care Companions. It will be initially part time with view to becoming a full time position.
You will build strong relationships with customers, families and colleagues, ensuring care remains personalised, responsive and of the highest quality. You will also support recruitment, induction and training to help maintain a skilled and engaged workforce.
No two days are the same. One day you may be carrying out a customer review in the community, the next delivering induction training, interviewing prospective Care Companions or supporting the office team with care planning and quality assurance.
Key ResponsibilitiesCustomer Support & Quality
- Carry out regular customer reviews and wellbeing visits.
- Develop and update person-centred care plans and risk assessments.
- Monitor the quality of care provided and identify opportunities for improvement.
- Build positive relationships with customers and their families.
- Respond to concerns or changes in customer needs promptly and professionally.
- Support hospital discharges and transitions into live-in care where required.
Supporting Care Companions
- Provide regular supervision, coaching and ongoing support.
- Carry out probation, competency and annual performance reviews.
- Offer guidance, encouragement and practical advice to Care Companions.
- Assist with placement matching and introductions.
- Promote wellbeing and maintain high levels of engagement amongst Care Companions.
- Support investigations, incident reviews and reflective practice when required.
Recruitment & Training
- Assist with recruitment, interviews and selection of new Care Companions.
- Deliver induction training for new employees.
- Support mandatory training and ongoing professional development.
- Identify individual learning needs and encourage continuous improvement.
- Promote Guardian Live-In Care as an employer of choice.
Office & Operational Duties
- Maintain accurate customer and staff records.
- Update care plans, risk assessments and quality documentation.
- Work closely with the office team to ensure continuity of service.
- Participate in the on-call rota where required.
- Support service development and quality improvement initiatives.
- Ensure compliance with company policies and relevant legislation.
What We're Looking ForEssential
- Experience working within health or social care.
- Excellent communication and interpersonal skills.
- A compassionate, person-centred approach.
- Strong organisational and time management skills.
- Ability to work independently and as part of a team.
- Good IT skills.
- Full UK driving licence and willingness to travel throughout Scotland.
- Flexibility to work occasional evenings or weekends when required.
Desirable
- Experience supervising or mentoring staff.
- Experience delivering training or inductions.
- Knowledge of live-in care services.
- SVQ Level 3 (or above) in Health & Social Care or equivalent.
- Understanding of Care Inspectorate standards and person-centred care.
- Experience of recruitment or interviewing.
