Job Summary:
The Healthcare Assistant will support the nursing, MDT and clinical teams by performing a range of clinical skills to support chronic disease management, long term conditions, QOF/ patient recalls and patient health promotion to deliver prescribed programmes of patient care.
You will work under supervision of the Nurse Partner Leads, Nurse Manager and Nurse Deputies to contribute to the smooth and effective running of clinical services across all sites.
The role involves direct patient facing care, will require accurate documentation in patients’ records, preparing and maintaining clinical environments including adherence to infection control and protection.
The post holder will provide high-quality clinical patient care to the highest professional standards, ensuring strict confidentiality and adherence to all practice guidelines, policies, and protocols at all times. This includes HCA, mandatory or practice training and attendance at nurse meetings.
Job Responsibilities may include:
Clinical:
- Support achievement of QOF indicators and targets
- Undertake new patient health checks
- Assist with Diabetic Part 1 reviews
- LD/ SMI reviews
- BP monitoring
- Urinalysis management
- Phlebotomy including drug monitoring (DMARDS)
- INR star management of results (Warfarin)
- Basic wound care/ removal of stiches
- Assist with minor surgery
- Administering flu, shingles and B12 injections (with PSD)
- Height and Weight monitoring
- Restocking/maintenance of equipment
- Restocking of clinical areas
- Restocking of clinical rooms
- ECG recording
- Smoking cessation
- Chaperoning duties when required
- Vaccine/cold chain storage, monitoring and recording of temperatures
- Preparing and maintaining environments and equipment before, during and after patient care interventions including assisting GPs during the performance of minor operations
- Assisting in the assessment and surveillance of patients’ health and well-being
- Undertaking specific clinical activities for named patients that have been delegated and taught specifically in relation to that individual
- Helping to raise awareness of health and well-being and how it can be promoted
- Assisting with the collection and collation of data on needs related to health and well-being
- Any other delegated duties appropriate to the post
Administration and professional responsibilities:
- Participate in the administrative and professional responsibilities of the practice team
- Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes
- Ensure the clinical computer system is kept up to date with accurate details recorded
- Ensure appropriate items of service claims are made accurately, reporting any problems to the practice administrator
- Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice
- Ensure collection and maintenance of statistical information required for regular and ad hoc reports
- Attend and participate in practice meetings as required
- Assist in formulation of practice philosophy, strategy and policy
- To assist in seasonal and special projects as requested e.g. flu campaign
- To maintain appropriate registration and training requirements appropriate to your role
- To maintain any mandatory and statutory training and attend any training days as requested by the practice
- To attend practice or ICB training events
- Attendance and participate in team meetings or discussions with the aim of maintaining or improving nurse services for patients
- Ensure you wear the appropriate PPE at all times adhering to Infection Prevention and Control (IPC) to include wearing clean and appropriate nurse uniform/ dress code
