We are seeking a dedicated Healthcare Assistant to join our team. As a Healthcare Assistant, you will play a crucial role in providing individualized care and support to residents in line with their care plans and needs. You will work closely with a multidisciplinary team to ensure the well-being and comfort of our residents.
Duties:
- Engage with and follow residents' care plans, providing personalized support tailored to their individual needs.
- Update care plans and risk assessments accurately and in a timely manner, adapting practice to meet changing needs.
- Utilize IT systems and tools to maintain residents' files and capture relevant information.
- Offer options that empower residents' choices, emphasizing appropriate risk-taking aligned with care plans and goals.
- Capture outcome information and data using documentation processes to support reviews and discussions.
- Complete delegated tasks within set deadlines and quality standards, reporting back to senior staff as required.
- Support residents with mobility issues, assisting with transfers and hoisting when necessary.
- Assist residents with daily personal care routines, continence management, and medication administration following provided training.
- Provide key worker support, review resident information regularly, and liaise with external care professionals as needed.
- Act as an advocate for residents during formal meetings or critical situations.
- Collaborate with colleagues, families, friends, and care professionals to ensure comprehensive support for each resident.
- Address service delivery issues, comments, and complaints according to policies and procedures.
- Ensure resident safety by adhering to internal policies, medication management systems, and regulatory requirements.
- Prioritize health and safety for oneself, residents, and visitors at all times.
Requirements:
- Proficient in English to effectively communicate with residents, families, and colleagues.
- Experience in home care or care home settings is advantageous.
- Ability to drive may be required for certain tasks or responsibilities.
- Competency in IT systems for documentation purposes is desirable.
