Role Type: The role is 40 hours per week, working Monday to Friday from 8:00am to 5:00pm, with a requirement to provide on-call cover for two evening or weekend shifts per month.

About The Role
As a Care Coordinator, you will play a vital role in ensuring that our service users receive safe, effective, and person-centred care. You will be responsible for coordinating care visits, managing rotas, and acting as a key link between our care team, service users, families, and healthcare professionals.
Key responsibilities include:

Planning, coordinating, and scheduling care services to ensure continuity and high standards of care for our service users.

Using our electronic scheduling system, to create and maintain efficient staff rotas, ensuring care workers receive their schedules in a timely manner.

Monitoring care delivery and responding proactively to changes in service user needs, staffing requirements, and operational demands.

Producing accurate reports and analysing scheduling data, including missed, late, and adjusted calls, to support service performance and compliance requirements.

Providing hands-on care support when required, ensuring excellent standards of care are maintained at all times.

Participating in the on-call rota, and responding to operational issues outside of normal working hours.

Liaising with local authority brokerage teams and other external agencies regarding new care packages, changes in care requirements, and service provision.

Acting as a primary point of contact for service users, families, care staff, healthcare professionals, and social care teams regarding care packages and scheduling queries.

Monitoring system alerts, identifying potential issues, and escalating concerns appropriately to the Registered Manager.

Supporting the smooth day-to-day operation of the service while maintaining exceptional levels of customer service and care delivery.

You will have:

Previous experience in a customer service, care coordination, scheduling, or call-handling role.

Experience planning workloads, managing schedules, or creating staff rotas within a busy environment.

Excellent organisational skills with the ability to prioritise competing demands and work effectively under pressure.

Strong IT skills, including experience with Microsoft Office applications. Knowledge of Nourish or similar care scheduling software would be advantageous.

Exceptional communication and interpersonal skills, with the ability to build positive relationships with colleagues, service users, families, and professionals.
