About Changing Lives through Changing Minds
Changing Lives through Changing Minds is a growing charity supporting children, young people and families from our base at Manor Farm in Coleby, North Lincolnshire.
Everything we do is built around strong relationships, compassion and evidence-based practice. Through a neuro-affirming, trauma-informed approach, we help children, young people and families better understand themselves, overcome challenges and achieve positive outcomes.
As our neurodiversity services continue to expand, we're looking for an experienced SEND Practitioner to join our team.
About the Role
This is a rewarding opportunity to support children, young people and families following a diagnosis of ADHD.
You'll hold your own caseload, supporting children and young people who have recently received a diagnosis and helping them, alongside their families, develop practical strategies to move forward with confidence.
Through focused, short-term interventions, you'll help children, young people and families improve emotional wellbeing, participation and everyday life.
Every family's journey is unique.
One child may need support with emotional regulation or anxiety, while another may benefit from strategies around executive functioning, sensory processing, communication or self-esteem. Parents and carers may need guidance to better understand their child's diagnosis, while schools may value practical advice on creating more neuro-inclusive environments.
You'll work alongside each family to understand their individual strengths and needs before developing tailored intervention plans that help children and young people thrive at home, in education and within their wider community.
This is a varied role where you'll work directly with children, young people and families, while building effective partnerships with schools and other professionals to provide joined-up, holistic support.
What you'll be doing
Your work may include:
- Holding and managing a caseload of children and young people following a neurodevelopmental diagnosis.
- Designing and delivering neuro-affirming interventions tailored to the individual needs of children, young people and families.
- Supporting children and young people with areas such as emotional regulation, anxiety, executive functioning, sensory needs and social communication.
- Empowering parents and carers to better understand their child's diagnosis and develop practical strategies at home.
- Working collaboratively with schools, families and partner agencies to achieve positive outcomes.
- Providing advice, consultation and guidance to educational settings where appropriate.
- Completing assessments, reports and professional documentation.
- Contributing to a supportive, innovative and evidence-based service.
We're looking for someone who:
- Has experience supporting neurodivergent children, young people and families.
- Has experience supporting autistic children and young people, those with ADHD, and understands neuro-affirming practice.
- Understands child development, attachment and trauma-informed practice.
- Can assess individual needs and develop practical intervention plans.
- Builds trust and positive relationships quickly with children, young people and families.
- Enjoys working collaboratively with parents, schools and professionals.
- Has excellent written communication and report-writing skills.
- Can confidently manage their own workload and caseload.
- Is compassionate, adaptable and emotionally resilient.
- Is committed to achieving positive outcomes for children and families.

Please Note
This role is available on either a Full-Time basis (from £27,982.50) or a Term Time Only basis (39 weeks + 2 weeks, from £24,726.15). Please indicate your preferred option when applying.
The role is based at Manor Farm, Coleby, North Lincolnshire.
The organisation is located within a rural countryside setting, so candidates should be comfortable travelling to this location.
