We are seeking an experienced and dedicated clinician to join our Community Mental Health Team (CMHT).
In this vital role, you will work with adults (aged 18+) across a diverse caseload.
You will provide high-quality, patient-centered care, with a particular focus on supporting individuals of all ages experiencing memory difficulties, as well as those navigating functional disorders such as depression, anxiety, and psychosis.
As a valued senior practitioner within our collaborative multidisciplinary team, you will have the opportunity to shape care pathways, utilise your clinical expertise, and directly improve the wellbeing of individuals within our community.

Assessing, treating and supporting those with mental health conditions/suspected MH conditions in the community
Supporting service users to function to their optimal ability in their environment. This can involve working in a diverse range of settings with a range of associated demands. For example: challenging behaviours and/or exposure to poor hygiene (personal/environmental).
Maintaining and promoting a safe environment taking account of infection control and identifying and reporting hazards and risks in a timely manner.
Accompanying, escorting and transporting service users as required.
Working contracted hours as per the requirements of the service. May also be required, at times, to work at other Trust units and/or sites.
Physically able to carry out all duties required of the clinical setting, including successful completion of physical interventions training as required

To contribute to the planning and implementation of interventions.
To monitor clients’ progress, reporting on clients’ psychological, physical, behavioural and social needs.
To adapt and evaluate interventions ensuring that they support the client and his/her carer to meet their needs, adjusting own approach accordingly.
To act as lead professional/Care co-ordinator to designated caseload/provide interventions as required
The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
