Responsibilities
- Deliver tailored support to adults with diverse needs, ensuring their safety and well-being at all times
- Assist with daily activities such as personal care, meal preparation, and medication management according to individual care plans
- Support clients in engaging with community activities, promoting social inclusion and independence
- Maintain accurate records of support provided, including updates to care plans and progress notes
- Communicate effectively with clients, families, and multidisciplinary teams to coordinate care and address needs
- Drive clients safely to appointments or outings when required, ensuring adherence to road safety protocols
- Monitor and report any changes in clients’ health or behavior promptly to relevant professionals
Requirements
- Experience working with children or adults requiring support is highly desirable
- Valid driving license and ability to drive safely for client transportation responsibilities
- Strong childcare or caregiving background with understanding of care plans and individual needs
- Excellent communication skills to build trusting relationships with clients and team members
- Proficiency in IT systems for record keeping, reporting, and communication purposes
- Ability to work flexibly across different shifts, including evenings or weekends if needed
- Compassionate attitude combined with active listening skills and patience
