Working within the Integrated Operations Department, you will join the Homeless Intermediate Care Team (HICT), part of the North Central London (NCL) Out of Hospital Care Model (OOHCM). The OOHCM provides health and social care support, alongside temporary accommodation, for people experiencing homelessness following discharge from hospital. The service aims to break the cycle of individuals being discharged back onto the streets, leading to repeated presentations at emergency departments.
The Homeless Intermediate Care Team supports patients discharged from UCLH, Royal Free, and Whittington Hospitals, including those placed within Discharge to Assess (D2A) Pathway 2 step-down beds. The team delivers case management, assessment, treatment coordination, and assertive signposting to community-based services for a time-limited period outside of the hospital setting. The service works with adults who are homeless, rough sleepers, those at risk of rough sleeping, and individuals with no recourse to public funds (NRPF).
You will support the team with the administrative duties to support our team and our patients.
As an Administrative Assistant, you will play a key role in supporting both the clinical and operational functions of the service. You will contribute to the effective delivery and day-to-day coordination of HICT, ensuring high standards of administrative support and service delivery across the multidisciplinary team.
The role involves working within a multidisciplinary environment, supporting the delivery of a holistic, wraparound model of care for homeless individuals across North Central London. Responsibilities will include coordinating meetings and MDT activity, managing referrals and service administration, maintaining accurate records, and supporting communication between internal and external stakeholders.
