We are looking for a Social Prescriber who understands that health is about much more than medicine. This is an opportunity to use your experience to help people address the practical, emotional and social issues that affect their wellbeing.

As a Social Prescriber, you will work closely with community organisations, helping people access local services, reduce isolation, improve their wellbeing and take greater control of their health. Every day is different, and every conversation has the potential to make a real difference.
You don't need to have worked in healthcare before—we're looking for compassion, resilience, excellent communication skills and a genuine desire to help people live healthier, happier lives.
Travel is essential, working between two practices, providing clinics/support to patients as needed.

7.5 - 12 hours per week (flexible working pattern)

£15.38 ph (£30,000 pa pr)
Contract
Permanent - subject to 6 month probation period

Duties and Responsibilities
- Social prescribers can refer patients to local services that support a patients’ social, emotional, or practical needs. They work with the patient to understand what could help and find the best support available in the local community ie;
- Support with finance/budgeting
- Food vouchers
- Baby food bank
- Low-income support
- Help complete forms ie – PIP, finance, parking applications
- Student finance England
- Signpost to social groups to meet new people – loneliness, cultural groups

- Carers Champion – offering support and advice
- To keep accurate computer records of all patient contact
- To attend meetings as required and those appropriate to the post
- To participate in such other duties and responsibilities as may from time to time be reasonably assigned in accordance with training received both internally and externally.
- To carry out any training necessary to perform this role on a regular basis, including any update training to ensure competencies maintained.
Please note: Full training will be given if required, supported by external Social Prescribing training courses.
