The Integrated Frailty Pathway MDT Co-ordinator will ensure that frailty multi-disciplinary meetings are co-ordinated efficiently and effectively, patient pathways are actively tracked to meet waiting time standards, support the production of evidence for national activity reporting.
A 12 month secondment opportunity has arisen for an MDT Pathway Coordinator within Integrated Frailty, working across RSCH Guildford and Milford sites.
Hours per week: 37.5 | Monday – Friday | worked, as required, between 08:00 and 18:00

This position is part of the team providing administration support to our Integrated Frailty clinical teams. The main responsibilities are:
Onboarding/discharging patients referred to the Frailty Same Day Emergency Care (FSDEC) and Frailty Virtual Ward (VW) using the Hospital Electronic Records system
Coordinating communications between patients, their carers’ and healthcare professionals across primary, hospital, social and community care services
Tracking patients’ care pathways to ensure frailty waiting time targets are met.
Developing, supporting and coordinating Multi-Disciplinary Team (MDT) Meetings.
Preparing information to report on activity within the FSDEC and VW
A significant level of liaison with different departments is required and the successful candidate needs to have highly developed diplomatic and influencing skills. Attention to detail, excellent communication and organisational skills are key to success in the position. Knowledge of EMIS, Millennium EPR and medical terminology, especially relating to frailty, is essential.

In accordance with FSDEC and VW Standards track all patient pathways referred into the Trust from assessment and/or decision to treat through to treatment by interpreting patient notes, pathology and multi-disciplinary team meeting notes
To work as part of the wider Integrated Frailty Pathway services including FSDEC, Urgent Care Response (UCR), the Virtual Frailty Ward (VFW) and Planned Care
workstreams in order to deliver a seamless transition through services
To closely liaise with all frailty clinical teams highlighting delays in specific patient pathways and agreeing actions and administrative service improvements required to address delays.
Arrange and co-ordinate weekly held MDT meetings and check they are run in accordance with standard operating procedures
Collect frailty data in accordance with Trust and and NHS England target requirements. Ensure confidentiality and security of data in accordance with organisational requirements and in line with the data protection act.
Liaising closely with Consultants, Clinical Nurse Specialists, Senior Nurses, Secretaries, and hospitals referring patients to the MDT, to identify which patients are to be discussed at MDT meetings. Distribute the weekly agenda to all departments in the MDT structure via email. Ensure relevant case notes, results, x-rays, previous MDT pro-formas etc are available for the weekly Multi-disciplinary team meetings.
Prepare the pro-forma documentation for the MDT meeting from information extracted from patient notes, pathology and radiology reports and file a copy of this in the patients file after the meeting when a treatment decision has been made.
Keep accurate Minutes of what has been agreed at the MDT meeting and maintain an efficient filing system to ensure prompt and easy access to all the agreed patient pathways.
Develop and maintain effective working relationships with other departments and other Trusts. Ensure all referrals as a result of the MDT are actioned in accordance with Trust standards.
To facilitate and assist MDT meetings using conferencing equipment linking Trusts as necessary.
To ensure all equipment in the RSCH MDT rooms is in working order, and inform management that working day if found not to be in working order.
Produce and keep updated operational procedures for own MDT to facilitate effective leave cover.
Contribute towards the professional development and training of other MDT Co-ordinators and associated staff employed by the Trust
To work as part of pre-existing Integrated Frailty Crisis Pathway including the VFW, Frailty FSDEC, Urgent Care Response (UCR) and Planned Care workstreams in order to deliver a seamless transition through services
To develop and maintain effective working relationships with all relevant organisations across the Guildford and Waverly Health and Care Alliance.
To provide cover for other MDT Co-ordinators.
To attend Trust and Alliance meetings as required.
To plan and organise own work schedule in order to meet deadlines and reporting requirements.
To use email and in house systems (such as patient administration and remote patient monitoring systems/platforms).
To ensure confidentiality and security of data in accordance with organisational requirements and in line with the Data Protection Act.
To prepare reports as required by senior staff within the Integrated Frailty Pathway services.
Book annual leave, study and professional leave and ensure dates are circulated to relevant departments.
To provide limited cover for absence for other MDT’s as appropriate and possible within time available.
To inform management of any data or organisation issues that may arise
