We're looking for a compassionate, proactive and organised Care Coordinator to join our friendly administration team at The Leith Hill Practice.
This isn't just another admin role. Every day, you'll be helping patients with long-term health conditions and complex care needs access the right support, at the right time. You'll be the person who keeps everything running smoothly behind the scenes while making a real impact on people's lives.

What you'll be doing
As part of our administration team, you'll work closely with GPs, nurses, healthcare professionals and community services to coordinate patient care and ensure every patient receives the support they need.
Your day could include:
- Coordinating appointments, reviews and personalised care plans for patients with long-term conditions.
- Being a friendly point of contact for patients, carers and healthcare professionals.
- Working with clinicians to ensure patients receive the right care at the right time.
- Liaising with hospitals, community teams, social care and other external services.
- Monitoring patient pathways, following up where needed and helping reduce missed appointments.
- Maintaining accurate patient records and clinical registers.
- Supporting important areas of care including Learning Disabilities, Mental Health, Palliative Care, Cervical Screening and Safeguarding.
- Helping patients access information and services that empower them to manage their own health and wellbeing.

- Understands the importance of confidentiality and professionalism.
It would be a bonus if you have:
- Experience in a healthcare, GP practice or NHS setting.
- Knowledge of SystmOne (TPP).
- An understanding of community services or personalised care.

Essential requirements
- Excellent communication and organisational skills.

- Full UK driving licence and access to a vehicle.
